1. The Clinic encourages you to book your appointments through Patient Ally, our patient portal. Available appointment times are displayed live on the platform. Additionally, Patient Ally allows you to view your lab results and summaries of your clinic visits.

2. If you do not yet have a Patient Ally account, please complete this form so that the Clinic can assist you in setting one up.

Note: You should only create a Patient Ally account when prompted by the Clinic. The Clinic staff needs to initiate the creation of your Patient Ally account in order to link it appropriately to our Clinic.
