We accept Zelle, Venmo, Visa, MasterCard, and American Express. In office, we also accept cash payments.
Card Payments
Important: If you are making a payment on behalf of someone else or if the payment account does not match your name, please provide the full name and date of birth of the individual for whom the payment is being made. Additionally, it would be helpful to specify the purpose of the payment, such as "copay."

For patients enrolled in an Allied Pacific IPA HMO plan, we currently provide primary care services only. At this time, we are not authorized to offer subspecialty services such as Sleep Medicine under Allied Pacific IPA.

If you are an Allied Pacific HMO patient and Dr. Hasanuddin is your primary care physician (PCP), we would be happy to assist you with a referral for sleep as appropriate.
